I want to enable the "Check Deposit Service" for my mailbox account listed above. I agree and authorize Scan Mailboxes to add check deposit service subscription plan to my account for $5.00 per month or $60.00 per year (prorated) depending on the subscription plan I am currently on. I understand this service becomes available after the Open and Scan Request is completed containing any check(s). I understand that I am responsible for marking specific mail item containing check(s) to be deposited in my online mailbox account.The name of bank, endorsement name and account number I entered on this form is what will be endorsed on the back of my check(s) along with "FOR DEPOSIT ONLY" stamped.
I understand that I will be charged for the check deposit service if requested before 2 PM central time zone. Scan Mailboxes will deposit the endorsed check(s) based on the combined amount of the check(s):
DOLLAR AMOUNT BASED DEPOSIT:Under $1,999.99 If the total combined amount is under $1,999.99 — it will be shipped via USPS First Class Mail with tracking #. Your account will be charged $4.95 for the first check, add $1.00 per additional check within the same deposit.
For this reason, if the check is delayed or lost, you can use your mobile deposit app to make a check deposit from the scanned check. We strongly encourage you to try to use your mobile deposit app first before making a check deposit request.
